COTS’ Intake Specialist is an integral part of a multidisciplinary team responsible for collaborating with clients who are experiencing homelessness and assisting them in their journey to obtain housing. Roles and responsibilities include the client intake process, including a tour of the facilities, and a warm hand off to the client’s care manager. A successful Intake Specialist will have knowledge of COTS shelter policies, and guidelines as well as other programs and services offered at COTS. This position reports to the Associate Director of Shelter Services and is a permanent part-time (20 hours/week) position.

ESSENTIAL DUTIES 

  • Conducts intake interviews with clients to gather information and assess their needs, ensuring compliance with intake / screening procedures.
  • Completes necessary documentation throughout the client intake process.
  • Schedules new member orientation.
  • Provides tours of the Mary Isaak Center and Mary’s table after intake.
  • Prepares client for their first Care Manager meeting including a review of housing and care plan opportunities, including CalAIM services.
  • Assist in crisis intervention and conflict management as needed.
  • Enters intake data and assessment including the Coordinated Entry, and the HMIS standardized intake information into Efforts to Outcomes (EtO) database.
  • Verifies CalAIM eligibility after intake.
  • Responsible for complete, accurate documentation and data collection for each participant served.
  • Adheres to HIPAA security requirements standards at all times.
  • Attends and participates in staff meetings, trainings, and agency functions.
  • Other duties as assigned.

QUALIFICATIONS

  • Experience working with homeless individuals and other marginalized populations in crisis.
  • Strong written and verbal communication skills.
  • Basic interviewing and investigative techniques.
  • Ability to follow policies and procedures.
  • Ability to self-manage the completion of work responsibilities according to deadlines.
  • Ability to review and evaluate documentation for accuracy.
  • Ability to share client intake information in a way that is accessible to team members.
  • Basic proficiency in using Microsoft applications such as Excel, Word, and Outlook and an ability and willingness to learn new platforms and systems as needed.
  • Familiarity with the county HMIS system’s Efforts to Outcomes (EtO) database.
  • Bilingual in English and Spanish required.
  • Ability to work effectively with people of diverse cultures, ages, economic backgrounds, and abilities in a culturally competent and sensitive manner.

EDUCATION

Two years of related experience, or equivalent combination of education and experience. B.A. in Social Work or related field desirable.

LANGUAGE, MATH & REASONING SKILLS

Ability to read, analyze, and interpret documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations, including budgeting. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in written or diagram form and deal with several abstract and concrete variables.

This position’s primary location is 900 Hopper Street, Petaluma. Telecommuting is not optional in this role.
