NUTS + BOLTS

The Manager of Coordinated Care is a vital member of a multidisciplinary team responsible for the delivery of a variety of care management services. COTS service approach is focused on effectively combining services that address mental health, physical health, substance abuse, social determinants of health, and housing retention. These services will assist clients experiencing homelessness with finding permanent housing as well as assist clients that are stably housed to ensure they maintain their housing.

This position provides support and mental health services to clients experiencing homelessness, including underserved chronically homeless clients who have entered shelter services or a variety of supportive housing options in the community. This position supports individualized care and housing plans for clients and facilitates care coordination within COTS programs and community-based health and wellness partners. This position provides and assists with clinical care through one-on-one and group sessions for COTS clients and tenants, as requested. This position also assists clients and tenants in navigating healthcare services through hospital systems and community-based care providers.

ESSENTIAL DUTIES

  • Reviews client intake assessments, using clinical judgment, to identify and address clients’ needs in the areas of healthcare, mental health, and substance use disorders.
  • Develops and / or coordinates the development of clients’ service plans, based on the intake assessment, and accepted social work interventions.
  • Delivers, or monitors and coordinates the delivery of mental health services to clients related to behavioral care, including linking with other treatment providers.
  • Provides traditional counseling services and support to clients.
  • Provides clients with self-management skills and information needed so they can manage their health and housing.
  • Collaborates alongside the case management teams to ensure client care.
  • Collaborates with other departments to implement a Harm Reduction environment in COTS programs.
  • Oversees successful achievement of goals and objectives associated with Coordinated Care.
  • Prepares project reports, as requested by supervising staff.
  • Participates in scheduled meetings to review and evaluate the services provided to clients, identify opportunities to improve the services delivered.
  • Works as liaison and coordinates with current care partners to strategize and develop relationships with new care partners in mental health, substance treatment and medical care.
  • Collaborates with care team to structure clinically focused housing navigation, housing stability, and case management services.
  • Develops, maps, assesses, and maintains thorough knowledge of referral resources throughout the service area.
  • Develops and coordinates internship program in conjunction with Human Resources.
  • Develops staff training curriculum related to coordinated care best practices. Coordinates and facilitates select training for COTS employees.
  • Collaborates with grants department regarding funding opportunities and the related delivery of program reporting requirements.
  • Thoroughly, accurately, and promptly documents all services rendered in case management system for all clients.
  • Completes required documentation and paperwork in a timely manner, and in accordance with departmental standards.
  • Always adheres to HIPAA security requirement standards.
  • Ensures HIPAA security requirements and standards are being met by shelter staff and swiftly and thoroughly addresses non-compliance issues as they arise.
  • Other duties as assigned.

 QUALIFICATIONS

  • Able to work as a member of a multidisciplinary care team.
  • Experience working with vulnerable populations, ideally individuals who have or are currently experiencing homelessness.
  • Experience administering behavioral health Medi-Cal services highly desirable.
  • Significant experience with comprehensive assessment tools.
  • Knowledge and experience working with HMIS and ETO databases preferred.
  • Knowledge and experience working with co-occurring populations (chronic health conditions, mental health, and substance use).
  • Knowledge and experience with evidence-based practices, including trauma informed care framework.
  • Extensive knowledge and experience in the accurate assessment of medical, mental health, and substance use challenges.
  • Strong clerical/administrative skills including personal computer skills (Microsoft Office software).
  • Ability to establish and maintain cooperative working relationships with personnel at COTS and people from outside of the organization.
  • Cultural competency and commitment to clients.
  • Compliance with information and security policy and procedures.
  • Ethical and professional behavior.
  • Bilingual English-Spanish preferred.
  • Ability to travel to typical destinations, which may include medical facilities or social service agencies in Sonoma County.

EDUCATION

Master’s degree in social work or related field from accredited university is required.

CERTIFICATES, LICENSES, REGISTRATIONS

LCSW or MFT in good standing in the State of California with minimum two years post-licensure. Reliable transportation, current auto insurance, and acceptable motor vehicle report required.

LANGUAGE, MATH & REASONING SKILLS

Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations, including budgeting. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in written or diagram form and deal with several abstract and concrete variables.

The noise level in the work environment is usually moderate but can occasionally become very loud. The temperature is usually moderate, but outside elements may be a factor on occasion such as very hot or cold conditions.

This position’s primary location is 900 Hopper Street, Petaluma. Telecommuting one to two days a week is possible, based on workload, performance, and supervisor approval.
