NUTS + BOLTS
The Recuperative Care Specialist position provides intensive case management and health care coordination in a non-clinical setting, collaborating with hospitals and health providers to facilitate the quality care of highly vulnerable clients recovering from a recent illness, injury, or surgery. The successful Recuperative Care Specialist is highly motivated, dynamic, and passionate about equitable and accessible health care for people experiencing homelessness and supports clients in this program to achieve the goal of stabilized health and self-sufficiency; and supports their transition into shelter or housing. This position reports to the Lead Recuperative Care Specialist.
ESSENTIAL DUTIES
- Performs the client intake process, completes paperwork, and orients client to Recuperative Care.
- Accurately collects information relevant to clients’ condition, assures proper triage and disposition based upon each client’s level of ability.
- Assists clients in following treatment plans as set forth by their medical providers and other health coordination, such as scheduling medical appointments, setting up transportation, communicating with hospitals and health providers on behalf of clients, scheduling required TB test at program entry, and assisting with pickup of client prescriptions.
- Helps clients identify and work toward their health, housing, and income goals through case management activities.
- Identifies appropriate permanent housing options for clients (subsidized housing, Section 8, HUD-VASH, affordable and market rate housing) and offers support with housing applications.
- Secures documents clients need for housing, including government identification, income verification, and rental history.
- Connects clients to resources and services available in the community to promote personal growth and development.
- Refers clients to appropriate agencies for accessing health insurance, Cal Fresh, General Assistance, Social Security benefits, and other services with an aim to increase income.
- Enters client data and case notes into HMIS database, following HIPPA privacy standards.
- Refers eligible clients to the CalAIM program upon intake for Recuperative Care and Housing Tenancy and Navigation Services and submits Treatment Authorization Requests (TARs) via Partnership Health Plan’s online portal.
- Manages CalAIM enrollment status for the duration of the client’s stay as defined by CalAIM Community Supports guidelines.
- Works with facilities team to ensure the unit and clients maintain a clean and safe environment.
- Works with facilities and other RC team members to ensure the unit is organized, stocked with supplies, and in a condition conducive to healing.
- Creates a welcoming, inclusive, non-judgmental environment and safe space for clients.
- Performs client discharge duties, including bed turnover, laundry, and cleaning.
- Manages conflict or difficult situations constructively and seeks appropriate assistance.
- Seeks to update and maintain current level of knowledge relative to professional practice by participating in educational opportunities and in-service training.
- Delivers the evening meal service to the Recuperative Care wing.
- Conducts nightly Wellness Checks prior to the end of the shift to ensure clients are safe and accounted for.
- Provides coverage as needed for the Recuperative Care Community Liaison:
- Receives and processes referrals from partner institutions in a timely fashion.
- Assesses client condition and interpreting their medical and social information.
- Requests additional documentation from referring institutions when needed.
- Communicates clearly and professionally with partner institutions regarding acceptance or denial of referred clients.
- When necessary, articulates recommendations for alternative levels of care.
- Adheres to HIPAA security requirement standards at all times.
- Open to other duties, as assigned.
QUALIFICATIONS
- Previous experience in healthcare industry, preferably medical admissions/discharge, case management or other care coordination role that involves direct client care.
- Basic knowledge of medical terminology
- Experience recognizing and responding efficiently to medical emergencies.
- Experience working with individuals who are homeless, disabled, elderly, or from other marginalized populations in crisis.
- Experience with substance abuse and mental health disorders strongly preferred.
- Application of trauma-informed care, motivational interviewing, harm reduction practices and de-escalation techniques is a plus.
- Experience working in databases; HMIS (EtO) experience preferred.
- Ability to drive to destinations throughout Sonoma County.
- Extremely well-organized, strong time management skills and confident decision making.
- Strong written and verbal communication skills.
- Bilingual in English and Spanish preferred.
- Ability to work effectively with people of diverse cultures, ages, economic backgrounds, and abilities in a fair and sensitive manner.
EDUCATION
Associate or bachelor’s degree in related field (i.e., social work, public health) and at least one year of related experience, or equivalent combination of education and experience. Candidates in Nursing Assistant (CNA) program, Medical Assistant (MA) program or taking courses toward a career in healthcare are strongly encouraged.
LANGUAGE, MATH & REASONING SKILLS
Ability to read, analyze, and interpret documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations, including medication measurements. Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in written or diagram form and deal with several abstract and concrete variables.
CERTIFICATES, LICENSES, REGISTRATIONS
Valid Drivers’ License, access to an insured vehicle, and satisfactory annual Motor Vehicle Report, required. Certified Nursing Assistant (CNA) or Medical Assistant (MA) or relevant experience would benefit this role.
