Comprehensive Supportive Wellbeing Services

COTS provides case-management and other wrap-around services to individuals in multiple partnership locations throughout Sonoma County.

Studios at Montero (SAM) is the conversion of a 50 year old motel into 60-units of permanent supportive housing for chronically homeless individuals. We are thrilled by the opportunity to partner with Burbank Housing and the City of Petaluma on this project. This project is made possible by the support of the State of California Homekey program, Providence Health, the City of Petaluma, the County of Sonoma, and the Wells Fargo Foundation.

Vida Nueva, a 23 unit apartment complex in west Rohnert Park, offers subsidized housing to individuals and families who have low or very low incomes and who are homeless or at risk of being homeless. COTS provides supportive services to tenants.

Petaluma River Place is a 50-unit building in Petaluma that offers subsidized housing to individuals, families, and agricultural workers with low or very low incomes. COTS provides supportive services to residents in 15 of these units.
