Wraparound Services
COTS provides wraparound supportive services to eligible Medi-Cal members who are enrolled with Partnership Health Plan of California. Kaiser Medi-Cal members may be eligible for Recuperative Care services only.
Services include healthcare coordination, case management services, short-term post-hospitalization shelter and meals, recuperative care, housing location services, rental assistance, and housing sustainability services for individuals moving from homelessness to permanent housing.
Eligible individuals include youth under 21 years of age who are unhoused or at imminent risk of losing their housing, adults over 22 who are experiencing homelessness and who have one or more chronic conditions, adults with high emergency department utilization, and adults with serious mental health and/or substance use disorder issues.
Public agencies, community-based providers, and other community partners may make referrals on behalf of an individual or family interested in services by completing the online referral form below. COTS will respond to all referrals within three business days of receipt.
